I certify that the information provided in this application, my resume, and/or pre-employment interview(s) is complete and accurate. I authorize my former employers and personal references to answer inquiries made by World Harvest Church (“WHC”), and I hereby release all such parties, including WHC, its affiliated entities, officers, employees, agents and representatives, including consumer reporting agencies that it may retain, from any liability as a result of doing so, or of seeking or obtaining other information in connection with a background check. I agree that if, in the reasonable opinion of WHC, I have omitted material information or made misrepresentations, or if the results of any inquiries are not satisfactory, any offer of employment may be withdrawn or, if I have already been hired and am working, I may be disciplined or terminated.

In the event WHC requires applicants or others offered employment to undergo drug testing, I consent to such testing as a condition of further consideration for openings for which I may be qualified or considered. My refusal to consent will indicate my voluntary decision to discontinue my consideration for employment with WHC. I further understand and acknowledge that failure to pass any alcohol or drug testing may result in disqualifying me from consideration for employment with WHC, and I expressly release WHC and any third party agents or representatives involved in providing alcohol or drug testing services to WHC from any liability.

I understand and acknowledge that my employment with WHC is on an “at will” basis, meaning that either I or WHC may terminate the employment relationship at any time, for any reason or for no reason, except that WHC will not do so for any unlawful reason. I further understand and acknowledge that no one except the Chief Executive Officer is authorized to enter into a contract or agreement of employment with me for any specific period of time or offer me any benefits different than those generally available to other similarly situated employees as stated in WHC’s benefit plans and Personnel Manual, and that if any such agreement contract or is made, it must be made in writing and signed by the Chief Executive Officers and me.

If I am hired, I understand I will be required to complete all forms and documentation WHC requires for new hire processing. My failure to do so may result in withdrawal of any employment offer (or termination if I have already started to work). Depending on my position, I may be asked to submit to a job-related medical evaluation after being given a conditional offer of employment. My failure to do so may result in disciplinary action up to and including termination, as deemed appropriate by WHC.

I understand that this application will be retained for six (6) months, during which time it may be reviewed to determine my qualifications for open positions. After six (6) months, it will be my responsibility to complete a new application if I wish to be considered for other employment opportunities.

I have read the foregoing and I further understand that if I am employed, I will be provided with and agree to adhere to the policies and procedures of WHC.
